It is therefore critical to respond and report promptly to any incident that resulted in, or could have resulted, in injuries, illnesses, damage to health, or facilities.
Occupational Injury
Employees: Incidents, hazards, occupational illnesses or injuries must be reported to your Management Supervisor, who will then report it to HSEWB. Workplace Incident Report (WIR) is to be submitted to wir@yorku.ca. For additional information refer to the Incident Investigation Program available on YU Link. Students and Others: Incidents, hazards, illnesses or injuries are to be reported to your faculty or department, who will then report it to Risk Management. York University Incident Report (Non-Employee) are to be submitted to Risk Management Services. For additional information refer to the Incident Investigation Program available on YU Link. Critical injuries must be reported immediately by contacting HSEWB following notification of EMS (911) and Security Services (416-736-5333 or ext. 33333). Access the Critical Injury definition.
Non-Occupational Absence and Accommodation
A completed Medical Absence/Accommodation Certificate (MAAC) must be provided to the Employee Well-being Office via ewb@yorku.ca to clarify abilities, restrictions, limitations and duration of absence. Policies, procedures, and forms pertaining to non-occupational absence, leave, and accommodation can be found on YU Link. Family Status Accommodation
A completed Request for Accommodation - Family Status must be submitted to Employee Well-Being via FAMSACC@yorku.ca advising of the reason for the request and detailing the Family Status responsibilities. Details regarding the process and forms pertaining to family status accommodation can be found on YU Link. Workplace Violence and Harassment
Report any incidents of workplace violence or harassment that you have experienced or witnessed to your Management Supervisor, who will then report it to HSEWB. York University’s Workplace Violence Prevention Program, Workplace Harassment Prevention Program and other resources can be accessed on YU Link.
